Ok ill try to explain....
How would you make a midi/melody for an acapella?

Like...for every note the singer sings, a synth hits the same note...
Dont know how to explain it....

How do you do this, is it just by ear or is it a program or something??

No, if you're writing a lead for a vocal track that has an existing acapella, you don't have to hit the same notes. It just has to be in the same key and follow the same rhythm for it to sound good.

Let's say for the first bar, the vocal only hits the notes C and E, you could use chord of C Major (C,E,G) for example. You could even go outside that chord as long as it's still in key.

Yes, it's something you do by ear. Something is either in key or not, and the only thing you can trust is your ears.

However, you can use software like melodyne/cubase variaudio to determine what notes the vocal is hitting. From there you could use music theory to determine which chords would fit.

Easiest way is to find a bassline that sounds in key with the acapella, then figure out your chord progression.
